The IRS is embroiled in controversy after officials apologized for the improper targeting of conservative groups seeking tax-exempt status. A number of Tea Party groups have come forward and charged the IRS unfairly targeted them for added scrutiny, requiring them to answer a host of questions about their activities.

Those interested in contacting the committee can fill out a form on the page, where they will provide information describing their dealings with the IRS. Users provide their names, when they submitted their applications, and whether they were approved. The page also asks if they have been audited by the IRS, details of the examination, and whether they ended up having to pay any back taxes or penalties as a result of the examination.

The committee also asks for permission to share that story publicly.
